Please complete the following form to apply to register as a patient of the University of Hertfordshire.

Please include your NHS number if possible which you should be able to find by visiting https://www.nhs.uk/nhs-services/online-services/find-nhs-number.

If you are moving to University accommodation you must register with the medical centre. This is a GP surgery based on the College Lane campus which will provide you with NHS medical services while you are at university. This should ideally be done before you arrive or as soon as possible. Please note you will then be registered at the UH Medical Centre while you are studying and living in Hatfield (NHS rules state you can only be registered at one GP practice at a time and should be registered where you sleep.) You should be registered here while studying so that you can receive medical attention when required, failing to do so may result in delays.

You can register as soon as you know the address of your University lodgings.

If you have ongoing medical problems, please contact the medical centre during your first week at university.

PLEASE CHECK WITH YOUR DOCTOR TO ENSURE YOU HAVE RECEIVED ALL RELEVANT VACCINATIONS BEFORE COMING TO THE UNIVERSITY, IN PARTICULAR MENINGITIS ACWY AND MMR BOOSTERS.
